San Juan County Public Health is currently in the process of vaccinating our citizens in accordance with the Governor’s Vaccination Distribution Program (https://coronavirus.utah.gov/vaccine/).
We are currently taking appointments for those individuals who are 18+ of age who reside in San Juan County, Utah. You can submit an appointment request by submitting the attached form, or by having someone you know enter in the information for you. Here is the link to register for an appointment: REGISTER HERE
San Juan County will be using the Vaccine Utah System to sign-up and make appointments. Vaccine Utah is a secure, web-based application where you will be able to go in, register for the vaccination and create an appointment.
